Ready to enroll? You’ll need to complete a few forms to get started. You can find everything you need right here:
- Completed Registration Form (Daycare)
- Copy of Birth Certificate
- Copy of Shot Records
- Signed Suspension and Expulsion Policy
- Health Record signed by a Physician
- Emergency Consent Form
- Permission Form (for children under 16 months to sleep in a baby bed or playpen)
- Pre-K Registration Form
